Ok I think I finally figured out a combination that works. Encode with H264 or x264 as AVC1 video and aac audio using an mp4 wrapper that writes UTC header codes and use 1280x720 or 1920x1080 resolution. If you use handbrake, use one of the Apple profiles to get the UTC headers. Otherwise, you can just run your mp4 through Avidemux and use mp4v2 muxer and 'copy' for both audio/video. If you are using virtualdub w x264 and change the fourcc in the settings to use AVC1 and pcm audio, then run that avi through, avidemux and encode to MKV, using video copy and audio to aac (Faac).
